Custodianship Declaration
If a minor child will study in Canada without parents, both parents (outside Canada) and a designated custodian (in Canada) must fill custodianship forms and have them executed in front of a notary.

The two forms are available
from this Government of Canada site. They must be downloaded and opened with Adobe in order to be viewed.

Filing them is
very straightforward.

The notarization of the forms is similarly hassle-free. Simply click on the button at the bottom of this page to book a video notarization appointment.

If given an option in the booking scheduler, be sure to select "
Notary Public".

You can book a single appointment for everyone, with different persons joining the video appointment from different locations; or you can book one appointment for the parents and another for the custodian.

A notarial fee of $45 will apply to the first document. The notarial fee for the second document will be $35.

Do not sign the forms until you are in front of the notary public by video conference.
